Child Abuse and Criminal Record Check. This is a link that takes you to a Pennsylvania State web site where you can download these 2 clearance forms (CY-113 and SP4-164). Please review the Band Booster Bylaws to determine if you are required to obtain these clearances. Currently, Mars Area Band Boosters Inc is following Mars Area School District policies regarding chaperoning and clearances. MASD policy, at the time of this writing, required drivers license scans only. Forms CY-113 and SP4-164 are not required by MASD for normal day-to-day interaction with students in the District. Mars Area Band Boosters Inc therefore only requires drivers license scans as performed by MASD and will accept the results of these scans for MABB Inc purposes UNLESS you will be chaperoning an event or activity requiring an overnight stay, in which case forms CY-113 and SP4-164 will be required.

Fund Raiser Request Form. Mars School District document. The District approves all fund raisers and requires that this form is filled out and submitted to the High School office for all fund raisers prior to the start of the fund raiser.

Parent Information collection Form. Use this form to provide your contact information to the Band Director and the Band Boosters. We will use this information to contact you in the event of an emergency and to convey Band and Band Booster related information to you. The information on this form will be distributed to the Band Director, Band Booster Officers and certain committee Heads as necessary.
Parent Orientation and Information Handbook. This booklet contains information that a parent of a new band student will need to know. Much of this information is contained in other documents or other places on this web site, but is condensed and summarized here for easy reference. Some of the information cannot be found anywhere else and can be considered 'common knowledge' by parents that have been involved with the organization for a while.
